Social Circle Athletic Physical Day is scheduled for Monday, April 12th in the Activity Center from 3- 5pm. All student athletes will need a PHYSICAL FORM .  Pages 1 – 2 (History Form) must be completed and signed by their parent/guardian before arriving to Activity Center.
